How do I qualify for the StratiFi Health Cares Care Management and Medication Management program?

Your clinical history identifies you as a candidate. If you have a chronic condition such as diabetes, pre-diabetes, hypertension, cardiovascular disease, asthma, COPD, hyperlipidemia or have been in the hospital recently for any of these conditions, you are eligible to participate in the program.  Once identified, you will receive a phone call from our care coordinator or nurse care manager to introduce you to the program and offer you more information and next steps.

What services are included in the StratiFi Health Cares program?

Our services are tailored to meet your needs. Whether you are struggling with lifestyle changes, understanding your medications, or if you are having difficulty managing your health condition(s) your local care team is here to support you and your doctors’ treatment plan. Your goals and concerns are heard and supported with tracking tools and symptom management. We are here to help you successfully navigate your health care and make it easy for you. 

Who is my care team?

Your local care team consists of Nurses, Pharmacists, Dietitians, Social Workers,  Care Coordinators, Pharmacy technicians, and Referral Coordinators who are available to support you.

How will StratiFi Health Cares program benefit me?

The care team addresses you as a total individual collaborating effort that focus on helping you achieve an optimal level of health and well-being. We make sure you are involved in the decision-making process along the way. We are here to partner with you to help navigate the ever-changing health landscape and listen to your needs.  We focus on preventative care, identification of gaps in your self-care management, education on medications and how to take them, and resources for achievement of your personal goals.

How can I be successful in the program?

Participation is the key for any success in reaching your health goals. As you work with your care team, you will be able to interact and grow in the relationship which will help improve your health outcomes. This means participating in your treatment plan activities as well as using appropriate referral resources.

How much does the care management program cost?

There is no cost to you. Costs for the program are paid by your insurance carrier.

How does this program affect my health benefits?

Your benefits are not impacted at all when participating in the StratiFi Health Cares program. By having your personalized care team working with you and your PCP goals, you will be able to have a better coordinated plan to utilize your benefits to your advantage.

Am I required to select a primary care physician?

We encourage all participants to have a Primary Care Provider (PCP) who will monitor your care and be a resource to ensure you get the appropriate care at the appropriate time. Any member of your care team can assist you in finding a PCP in your area.

Can I visit a participating specialist without going through my PCP first?

Yes, but it is best to ensure your PCP is aware of the care you may need so that they can better assist in the coordination of your care. Your PCP can help direct you to a network specialist which will prevent delays in scheduling and can also prevent duplication of services.  Once enrolled in our program, you will want to keep your care team informed of any specialists you are working with in the treatment of your care.
